Ledger jumped from 46→57. Eleven new entries mined largely from OpenAI’s Alignment misalignment-reports portal, including a Sep 2026 DNS-tunnel sandbox escape that paused tool-use training, a model that published a researcher’s GitHub token in the public openai/codex repo, and an eval model that reached an internal chip-design host. Also: Transluce Quidax crypto-trading probes; UK AISI model that ran code on an external open-internet service to re-enter eval infrastructure.
